No. This is an extra service. You should still make an appointment with your primary care physician to complete your Annual Wellness Visit.

No. The health assessment is not a substitute for seeing your regular doctor. It is meant to enhance your usual care.

No. This assessment does not replace any home health care or other programs you receive through CarePlus.

Yes. They have medical licenses and receive training. CarePlus reviews them every 3 years to make sure they continue to meet high standards.

No. The Health and Well-being Assessment is not counted toward member rewards.

Durante la evaluación

If you qualify, you may get a letter from Signify Health. The letter will have a phone number for you to call and set up your in-home health visit. A licensed healthcare provider will visit you at home to complete your assessment. This visit includes checking basic health measurements like blood pressure and heart rate. During your health review, the healthcare provider may:

  • Go over suggested health screenings based on your age and medical history
  • Talk about your health conditions, prescriptions, and steps to help you stay healthy
  • Answer questions about your medications and how to take them safely

For the best results, we recommend you schedule a follow-up appointment with your primary care physician (PCP) to review your assessment. If you need help finding a PCP, CarePlus can assist you or you can use the herramienta "Encontrar un médico" on our website.

Your in-home health assessment usually lasts about 45 minutes to 1 hour.

Yes, you may have a family member, caregiver or support person with you during the assessment. This can be helpful, especially if they help you with your health needs or care planning.

Use this time to talk about your current health, future health goals, and any care you receive from other doctors. This helps the healthcare professional understand your full health picture and offer advice that fits your needs.

No, you do not need to undress for this visit. You may be asked to roll up your sleeve or take off your socks for certain checks, but you will stay clothed during your assessment.

After your assessment, you'll receive a short report with suggestions for topics to discuss with your doctor at your next visit. If you have a primary care physician (PCP), CarePlus will also share your results with them. This helps you and your doctor manage your health together. The assessment is an extra layer of support and does not take the place of your regular doctor visits. Based on your results, CarePlus may recommend helpful programs or resources. Your results will not change your health plan rates or benefits.

Yes, you will receive a summary of your assessment after your visit. If your assessment was done online or by phone, you will get your summary in the mail within two to four weeks. If you need another copy, or did not receive your results, call the number on the back of your CarePlus member ID card for help.

Yes. If you have a PCP, they will also receive your results.

No. You should continue to see your primary care physician (PCP) for routine checkups, your Annual Wellness Visit and ongoing care. If you do not have a PCP, call the number on your CarePlus member ID card or use the herramienta "Encontrar un médico"

It's important to review your assessment results with your primary care physician (PCP). Signify Health may call you after your assessment to help you schedule a visit with your PCP. If you do not have a PCP or would like to find a new one, you can call the number on the back of your CarePlus member ID card. A CarePlus Member Services representative can help you find a PCP in your network. You can also find a PCP through our herramienta "Encontrar un médico".

  • There is no charge for the assessment or any screenings conducted during it. One assessment per year is included in your CarePlus plan.
  • This service is supplementary and does not replace your Annual Wellness Visit with your provider. It does not affect your plan premiums or benefits.
  • Follow-up appointments or additional tests beyond the annual coverage may incur costs. Please review your specific plan details for more information.
  • CarePlus complies with all applicable federal civil rights laws and offers language assistance services at no cost.
