What are Special Supplemental Benefits for the Chronically Ill?
SSBCI are benefits offered on certain Medicare Advantage plans for people with 1 or more eligible long-term chronic health conditions who are at high risk of hospitalization or other adverse health outcomes and require intensive care coordination. These benefits are not part of Original Medicare. They’re meant to support a person’s health and daily needs.
Examples of what SSBCI may help with include:
- Food
- Utilities
- Transporte
- In-home support

- CareEssentials Allowance and Chronic Condition Care Assistance are a special supplemental benefit for members with specific chronic conditions who require intensive care coordination and are at a high risk for hospitalization or other adverse health outcomes. Qualifying conditions may include diabetes mellitus, cardiovascular disorders, chronic and disabling mental health conditions, chronic lung disorders, or chronic heart failure, among others. If you use this program for rent or utilities, Housing and Urban Development (HUD) requires it to be reported as income. Contact your local HUD office with questions.
